Directory Instructions:

1. Search by Location: Enter a city, state, or zip code into the search bar below labeled "Enter a location."  Select the mile radius of the location using the dropdown menu (i.e., 5, 10, 25, 50, or 100 miles).  Then click the blue "Search" button.
2.  Search by Name: Enter the full name of the individual into the search bar below labeled "Enter a location."  Do not enter the first name only or last name only.  For example, if searching for a clinician named Jane Doe, enter "Jane Doe," not "Jane" or "Doe."  Then click the blue "Search" button.
